The Ambulatory Surgery rotation has been established to give the resident a concentrated
exposure to the principles and techniques of patient selection and anesthetic care for, the
patient undergoing surgery and anesthesia in the outpatient setting. The rotation is for a
minimum of one-month duration during the CA-2 year and may be electively taken for
one or two month duration during the CA-3 year. During this rotation the resident will
learn the principles and techniques of patient selection and anesthetic care for, the patient
undergoing surgery and anesthesia in the outpatient setting. Post-operative analgesic
strategies, and management of post-operative nausea will be learned. A major emphasis
will be placed on regional analgesia use in the ambulatory surgery setting.
Goals for the CA-2 resident during Ambulatory Surgery Center Rotation:
- Learn the principles of patient selection for anesthesia and surgery in the
outpatient setting.
- Learn the principles of preoperative physical, laboratory and radiographic assessment of the patient for outpatient surgery.
- Learn the principles of prescription of an appropriate anesthetic technique for outpatient surgery.
- Learn and perfect general anesthetic techniques for outpatient surgery that will minimize the likelihood of an unplanned hospital admission.
- Learn anesthetic techniques that will minimize the occurrence of post-operative nausea and vomiting.
- Learn regional anesthetic techniques that will minimize the potential for unplanned hospital admissions.
- Learn monitored anesthetic care (MAC) techniques.
- Learn the principles of anesthesia for ophthalmologic surgery, including general anesthetic techniques, topical anesthetic techniques and retrobulbar and peribulbar anesthetic blocks.
- Learn the principles of the post-operative management of patients having surgery in the outpatient setting, specifically:
how to manage post-operative pain without excessive sedation;
how to manage post-operative nausea and vomiting without excessive sedation;
how to manage the post-operative care of the patient who has undergone regional anesthesia in the outpatient setting
- Learn how to assess the patient who has undergone general or regional anesthesia for outpatient surgery for adequacy of recovery and fitness for discharge from the outpatient surgery center.
- Learn cost-containment considerations for the administration of anesthesia in the outpatient setting.
- Become familiar with the administrative aspects and functioning of a free-standing outpatient surgery center
Goals for the CA-3 resident:
The senior level resident spending elective time at the ambulatory surgery center will be expected to expand the knowledge and technical base of the principles learned as a CA-2 resident. (See CA-2 Goals above). The resident will be expected to perform at a level that reflects mastery of all aspects of the administration of anesthesia in the outpatient setting
